MEET OUR TEAM
NPLCs are a new, innovative model for delivery of comprehensive primary health care in Ontario and Canada. The model is designed to improve access to care for the thousands of individuals and families who do not currently have a primary health-care provider. One of the unique aspects of the model is the incorporation of nursing leadership within an interprofessional team.
Valerie is a Primary Health Care Nurse Practitioner and Clinical Wound Specialist. Since graduating from the NP program at the University of Windsor in 2002, she has worked in Primary Care, Long-Term Care and Emergency Medicine. Currently, Valerie is the Executive Director/Clinical Lead NP at Twin Bridges NPLC and is grateful for the innovative and caring team that make up the NPLC in Sarnia.
Valerie has been involved with many funding initiatives at the local, provincial and national level in an effort to spotlight the important work that Nurse Practitioners do and to advocate for patients to have interprofessional team-based primary care in order to maintain their optimal health throughout the lifespan.
Valerie spends her time outside of work with her husband, children and grandchildren and loves to travel with friends.
Diana is a Nurse Practitioner who graduated from the University of Windsor in 2003. Before working as an NP, her background in Nursing was in the Emergency Room. In her practice she has a special interest in mental health.
In her spare time she enjoys wakeboarding and going to the beach.
Amy MacLean is a Primary Care Nurse Practitioner and has been a TBNPLC team member since 2014.
She has 23 years of nursing experience with special interests in primary prevention and management of chronic diseases. Amy completed her Bachelor of Science in Nursing at the University of Western and earned her Nurse Practitioner Certificate at the University of Windsor.
In her spare time, she enjoys spending time with her family outdoors in nature.
Lana is a Primary Healthcare Nurse Practitioner at Twin Bridges NPLC. Lana graduated from Western University with her BScN. She has worked in public health, vaccine preventable diseases, travel medicine, the emergency room and in private clinics as a Registered Nurse. In 2016 Lana completed her Masters in Nursing along with her Nurse Practitioner at the University of Windsor.
Lana has an interest in yoga and has her certification to instruct. Lana also enjoys paddle boarding and physical activity of any kind. She loves spending time with her children, husband and dog when she is not caring for patients of all ages at the clinic.

Rachel is a Registered Nurse who graduated with a BScN from Western University in 2012. She began her career working in both Emerg and Palliative Care. In 2014, she became apart of the Twin Bridges team and has been here ever since. Rachel finds the challenges of healing wounds interesting and enjoys assessing our pediatric population, helping moms as they navigate all the developmental stages.
On her days off she can be found with her kids and husband exploring the outdoors or working on a home project.
