Patient registration forms are now being accepted for new patient enrolment at TBNPLC.
Who can apply?
- Newcomers to Sarnia-Lambton who do not have a primary care clinician (NP or MD)
- Unattached persons who currently do not have on-going primary care from a NP or family doctor
How do you apply?
- Stop in to the clinic (153 Christina Street S) and pick up a patient registration form
- Complete the registration form online (must be printed and handed in to the clinic in person)
